Under Government Guidelines on COVID19 and to assist the NHS, we are collecting the following data for Track & Trace, should there be a need to provide this information to the NHS. Under Guidelines this information will be held for a short period and deleted. The information on this form is kept independant to our standard contact records and under our standard GDRP policies.
